Requirements to claim Fire Assistance Benefit:
1) Copy of certification from Bureau of Fire Protection
2) Copy of Barangay Certification. Proves to be that the house was involved in the fire.
3) Picture of the premises after the fire
4) Valid government issued ID.
5) All above 4 should be submitted within 60 days.
